  • Photo Permission Form

  • We sometimes display photos of our players as part of reporting what we do and our general and web site publicity. We only do this if we have permission.

    How do I view these forms ?

    Please click the links above if you want to to view the forms you need. Once you have them displayed on your computer screen you can them print them using your computer's printer.

    How do I print the form ?

      For PC users

    Right click on the link (a pop-up menu will appear)

    Scroll down the menu and click on 'Print Target'

      For Macintosh users

    Hold down the mouse button until the menu appears and then follow the instructions as per the PC user.

    Saving documents to your computer

    Internet browsers save documents in a variety of ways which can often be confusing so if you want to guarantee saving a document you can do the following:

      For PC users

    Right click on the link (a pop-up menu will appear)

    Scroll down the menu and click on 'Save Target As'

    You will then be prompted to choose a folder where you want to save the file.

    Once you've saved the file and you want to open it, go to the folder you selected and double click on the file name.

      For Macintosh users

    Hold down the mouse button until the menu appears and then follow the instructions as per the PC user.

    Adobe Acrobat Reader

    The forms here are in Adobe Acrobat Portable Document Files (PDF) format.

    Adobe Acrobat (PDF) files can be opened on both PC and Macintosh machines. To read them, though, you need the Adobe Acrobat Reader software. This can be downloaded free from their web site:-
